Dear Parents,
Today (3/9) from 3:30PM-5:30PM, there is a COVID-19 vaccination clinic in the Bound Brook High School cafeteria for students, staff, and community members ages 5 and up. The clinic is hosted by Green Brook Family Medicine. Children under the age of 18 will need a chaperone. All participants should bring a copy of their photo ID, insurance card, and COVID-19 vaccination card, if available.
Please complete and bring the attached consent form. One is for if your child is 5-11 years old and the other is for individuals 12 and up. We have also attached Spanish instructions on how to complete the form. Please see the attached flyer for more information about the clinic.
